Welcome to June, where we'll soon be hitting summer in the Northern Hemisphere, winter in the Southern. This month we will delve into Step 8 of the Ten Steps to Successful Breastfeeding, which reads, "Encourage breastfeeding on demand.”
There are several terms used for demand feeding, such as cue-based feeds, feeding on infant request (which happens to be my favorite, more polite term), and infant-led feeds. All, though, refer to the same behaviors by feeding whenever the infant shows readiness regardless of frequency. As you know, the frequency terminology used by Baby-friendly no longer refers to the number of hours (feed every 2-3 hours) but to eight or more times in a 24 hour period. Many birthing facilities now use the jingle, "8 or more in 24” to help staff remember feeding frequencies for client education purposes.
How often do exclusively breastfed newborns feed? Saki, Eshraghian & Tabesh (2013) state, "Despite numerous studies highlighting the advantages of exclusive breastfeeding during the first six months of infant life, there is little data on the daily variations in breastfeeding practices and patterns in terms of daily frequency (number of feeds) and cumulative suckling duration (minutes) among exclusively breastfed infants,” (p. 124). This study found, "Frequency of feeds was not significantly different between boys and girls in the daytime and the nighttime separately, but total frequency of breastfeeding during the twenty four hour period among boys was significantly higher than in girls,” (p. 127) and found the frequency to be between 6 and 18 times per 24 hour period. Extensive research indicates that not feeding frequently enough can lead to inadequate infant growth and decreased maternal milk supply, both of which can affect whether the parent continues exclusive breastfeeding for at least six months.
Whatever you decide to call demand feeding, it is one of the vital components to successful breastfeeding and begins at birth.
